Application Form

All potential applicants are required to take the eligibility assessment to qualify for consideration. If you are eligible for a grant, you will be forwarded to the application form.

The application package consists of three parts:
  • Proposal summary (online)
  • Application form (online)
  • Required supporting documents:
    • Detailed project budget
    • Organizational budget for current fiscal year
    • Board of Director list including affiliations
    • Most recent audited financial statements
    • 501(c)3 letter
Mail the proposal package to the following address. Proposal packages not received by SOAF by the stated deadline will not be considered.
